GoCardless Payment Details

Although your sessions are covered by your insurance company, we do require valid payment details on file. This ensures any shortfalls, excess payments, or non-covered sessions can be settled easily. Your details are securely stored through GoCardless.

Once complete this form will direct you to the Physiotherapy and Consent forms, these will take around 5-10 minutes to complete.    They are quite comprehensive to enable us to be prepared for your session and make your treatment as effective as possible.
