Upper Back Conditions

The Upper Back / Thoracic Spine
Upper back pain often develops from sustained postures, stress-related tension, disc irritation, or sudden injuries such as falls that may affect the ribs, and can lead to stiffness, discomfort, and reduced mobility. At Movement Perfected, we provide expert upper back pain physiotherapy in Moorgate and upper back pain physiotherapy in Marylebone, helping you identify and address the underlying cause of your symptoms rather than simply managing the pain. Common issues include postural strain, thoracic spine stiffness, rib dysfunction, and muscle tightness, all of which we treat using tailored, evidence-based rehabilitation programmes designed to reduce pain, restore mobility, and prevent recurrence.

Thoracic Stiffness and Joint Restriction
Thoracic stiffness occurs when the joints of the upper and mid-back lose their normal range of movement. You might notice this when trying to twist your body to reach for a seatbelt in the car, or when turning to look over your shoulder. This stiffness can lead to aching, tightness, and difficulty rotating or extending the spine, often felt between the shoulder blades. Stiffness may also contribute to neck or shoulder pain. Our Physiotherapy team focus on restoring thoracic mobility, improving posture, and helping the spine move more freely during daily activities and exercise.
Postural Thoracic Pain
Postural thoracic pain commonly develops from prolonged sitting, desk work, or sustained forward-rounded positions. Symptoms often present as a dull ache or fatigue across the upper back and between the shoulder blades. Pain usually builds gradually and eases with movement. MOVEMENTPEFECTED Physiotherapy addresses postural habits, spinal mobility, and strength endurance to improve comfort and reduce the recurrence of upper back pain.
Mid-Back Muscle Strain
A mid-back muscle strain occurs when the muscles of the thoracic spine are overloaded or fatigued. This may follow lifting, sudden movements, increased training, or prolonged static positions. Pain is often localised and tender, and may worsen with certain movements or deep breathing. Physiotherapy treatment with our team in Marylebone or Moorgate will aim to reduce pain, restore normal movement, and gradually strengthen the muscles to prevent recurrence.
Costovertebral or Costotransverse Joint Pain
These joints connect the ribs to the thoracic spine and play an important role in movement and breathing. Irritation in these joints can cause sharp or aching pain in the mid-back, often felt to one side and aggravated by twisting, bending, or deep breathing. Our Physios focus on restoring joint movement, reducing sensitivity, and improving rib and spinal mechanics.
Rib Dysfunction
Rib dysfunction occurs when one or more ribs are not moving normally in relation to the spine. This can cause localised upper back pain, chest wall pain, or discomfort with breathing, coughing, or certain movements. Symptoms can feel sharp or catching. Many people experience noticeable relief from rib dysfunction within two to six weeks of beginning physiotherapy, though exact recovery times may vary. Physiotherapy treatment aims to normalise rib movement, reduce pain, and restore confidence with everyday activities.
Thoracic Disc Irritation (Less Common)
Thoracic disc irritation is less common than disc problems in the neck or lower back, but it can still cause mid-back pain, stiffness, or referred symptoms around the chest wall. Pain may be aggravated by sustained postures or repeated movements. Physiotherapy focuses on improving movement strategies, reducing disc irritation, and supporting a gradual return to normal activity.
Breathing-Related Thoracic Pain
Breathing-related thoracic pain can occur when stiffness, muscle tension, or rib-joint dysfunction affects the movement of the chest wall during breathing. Pain may be felt during deep breaths, exercise, or prolonged sitting. Physiotherapy addresses rib mobility, thoracic spine movement, and breathing mechanics to improve comfort and efficiency. Pain Associated with Prolonged Sitting or Cycling Posture
Upper and mid-back pain is common in people who spend long periods sitting or cycling in sustained positions. Symptoms often include stiffness, tightness, or aching between the shoulder blades and may worsen as the day or ride progresses. Physiotherapy focuses on improving spinal mobility, postural endurance, and practical adjustments to reduce strain and improve comfort.
